Required Documentation for Participation

In accordance with NCAA bylaws, all students participating in athletics are required to provide proof of insurance (photocopy of front and back of card), to complete appropriate medical paperwork, and must be cleared by the Athletic Training Staff prior to their participation in preseason and/or regular season team related activities. 

Please note, the NCAA requires a physical within six months of the first date of practice. The 2019 practice dates are as follows:

* - Spring sports non-traditional segment generally gets underway in mid-to late September. An April or later physical is advised.

The medical paperwork is separate from what the Academy’s Health Center requires for a student to attend the Academy. Fall sports medical paperwork must be submitted by August 1st.
